Transition Planning

What are Vocational Rehabilitation Services?

Vocational Rehabilitation services are provided by The Division of Vocational Rehabilitation (DVR) agencies These services help prepare and enable individuals with disabilities to find and maintain jobs. The key purpose of transition-services is to ensure students move smoothly from school-life to adulthood.  Therefore, a student with an IEP should receive transition services to ensure that there is not a gap in service between leaving school and beginning services with DVR.


What is a Transition Plan?

A transition plan describes a course of study and related strategies and activities based on the student’s preferences, interests, needs and strengths (PINS), to help the student in reaching their postsecondary goals related to training, education, employment and/or independent living. The transition plan must be included in the student's Individualized Education Program when the student reaches age 14.  

                                    What are Transition Services?

Transition services are focused on improving the academic and functional achievement of a student with disabilities to enable the him or her to move from school to post-school activities. Post-school activities can include: college, vocational training, employment, continuing and adult education, adult services, independent living, and/or community participation. 

Transition services can be divided into the following activity or strategy areas:

* Instruction

* Related Services (Transportation and developmental, corrective, and other supportive services)

* Community Experiences

*Daily Living Skills

*Functional Vocational Evaluations
